Practicing Slow Dentistry The challenges facing dentists in these modern times are quite different from the ones their colleagues confronted in the past. The fast pace at which we live, an overwhelming number of appointments and procedures, a lack of managerial and organizational protocols with possible confusion at the practice, and uncomfortable levels of stress are some of the things that can detract from both excellence on the job and our general well being. Regardless of these observations, more and more professionals assume that this type of accelerated lifestyle, along with the constant need to grapple with time, is something…

VIEWPOINT continued from page 10 unanimously agree that working too hard for too long increases the risk of physical or psychological injury. Nobody can sprint around the track every single day of the week. Knowing how to balance the lulls in our practices with the bursts of activity is critical, not only when it comes to caring for ourselves, but also for being the best that we can be on the job. Worshipping Time Without a doubt, time is essential when it comes to enjoying practicing dentistry and making it beneficial for us and our patients. It seems far too…

FOCUS ON: Endodontic Emergencies Emergencies can be intrusive in a busy day! Q: How can the dental team distinguish between a true emergency vs a non-emergency? A: Much of how a dental team reacts to an emergency patient is based on training, judgment, and experience. There are 3 critical questions your dental team should be trained to ask an emergency patient in pain to protect an already fully scheduled day: How bad would you rate your pain? How long have you been in pain? Is there a stimulus that triggers your pain? Your team should carefully clarify each question so…

Risk for HPV-Related Throat Cancer Remains Low The prevalence of oropharyngeal cancers caused by the human papillomavirus (HPV) has increased in recent decades, with some groups much more likely than others to have the oral HPV infections that can cause these cancers. However, a study by Johns Hopkins Bloomberg School of Public Health researchers shows that the risk of developing HPV-related throat cancer remains generally low. HPV is transmitted to the mouth and throat mostly through oral sex and appears to cause about 70% of oropharyngeal cancers. While there has been interest in the possibility of testing for oral HPV…

An Efficient and Reliable Post and Core Technique Garry M. Radz, DDS INTRODUCTION Background Dentists frequently encounter patients presenting with teeth requiring endodontic treatment. When indicated by the clinical findings, the endodontic treatment is then followed by restoration with a post and core prior to preparation and placement of a definitive full-coverage crown restoration. Regardless of the materials and techniques available for predictable stabilizing and restoring endodontically treated teeth, the requirements for long-term function continue to include sufficient post and restoration retention, functional stress distribution, and minimal insertion stress.¹ Post and core restorations replace missing tooth structure to enhance the…

RESTORATIVE (taken at the beginning of the operative appointment before preparation), a temporary crown restoration was fabricated from a self-curing bis-acryl provisional composite (Luxatemp Ultra [DMG America]). This provisional material is known for its high flexural strength and long-term durability. The temporary was trimmed, polished, and cemented into place using temporary cement (TempoCem [DMG America]). The occlusion was finalized, and a final polish was imparted on the temporary crown by applying a liquid resin glaze (LuxaGlaze [DMG America]) to enhance the final aesthetics and stain resistance. Delivery of the Final Restoration When the patient returned to our office, the provisional…

Restorative Materials Continue to Evolve Spherical Supra-Nano Composites Peter Auster, DMD Dental practitioners are encouraged to use a composite material for the first time by an advertisement, a review, or word of mouth. Continued use of that restorative product is determined by the comfort zone the dentist attains with that material. Smoothness, a shade-blending effect for optimal aesthetics, nonstick handling, and an ability to polish easily are among the properties that clinicians look for in determining their go-to materials. THE IMPORTANCE OF FILLER PARTICLE DESIGN The size and shape of composite filler particles are vital to creating optimal success with…

DENTAL MATERIALS CASE 1 Figure 2. The patient presented with old deficient amalgam restorations on teeth Nos. 28, 29, 30, and 31. Tooth No. 30 was more undermined by recurrent caries than the others, but the patient requested a direct restoration at this point with an understanding that a crown (or onlay) would likely be required in the near future. Figure 3. The restorations were removed using a No. 556 bur in a high-speed electric handpiece (CAMOFfome System [Kavo]). The remaining decay was removed with a No. 4 round bur in a low-speed handpiece. Figures 4 and 5. Following the…

DENTAL MATERIALS resin was added to recreate the broken incisal and proximal edges of tooth No. 9 on the maxillary model. A palatal matrix was then created using a fast-set putty impression material (Virtual Putty [Ivoclar Vivadent]). This matrix is an indispensable aide in restoring Class IV incisal edge damage (Figures 10 and 11). The remainder of the old restoration was removed with a No. 556 high-speed carbide bur. The labial margin was established with an irregular long bevel to hide the marginal area using a coarse diamond (No. 15 uB [Microcopy]). Labial and palatal areas were then sandblasted (Microblaster…

RESTORATIVE market. The chemistry, packaging, and characteristics are analogous to ACTIVA cement. It is classified as an RMGI cement specifically designed for pediatric dental patients. The manufacturer claims that this material contains bioactive components that form hydroxyapatite, which integrates and regenerates the tooth structure.14 This material is used as a luting agent, as it has been shown to create bonds between the dentin of teeth and either stainless steel, pre-veneered, or zirconia crowns. The secure bond is created due to the vast amount of phosphate ions released from the material. The effect of this bond creates a passive eruption fit…
