Globalization: Realities for Dentistry With the effects of globalization impacting us, we will continue to experience a significant evolution/revolution within our profession. Fast-paced changes in how dentistry is delivered, and possibly defined, will challenge all involved with regard to professional business ethics, interprofessional relationships, and the long-held concept of an evidence-based practice as well as practice, laboratory, and material-related regulations. Much of the change is for the good, but sometimes it can be unsettling. For example, larger dental laboratories, under the pressure of international competition, are now marketing new ways to use certain dental materials clinically. High-tech fabrication processes with…

NEWS/TRENDS Yes, Virginia... continued from page 54 "On average, there's a 10% to 15% profit leak in a private practice," he said, "with much of that tied to money owed to the practice by patients or insurers. This is also why they are seeing a cash crunch." Commenting on the dilemma, a 47-year-old cardiologist from Philadelphia said recent steep 35% to 40% cuts in Medicare reimbursements for key cardiovascular services such as stress tests and echocardiograms have taken a substantial toll on revenue. "These cuts have destabilized private cardiology practices," he said. "A third of our patients are on Medicare.…

Losing Fat Can Improve Periodontal Disease Treatment A pilot study at Case Western Reserve University School of Dental Medicine indicates that humans can fight periodontal disease better if they have fewer fat cells. It involved 31 obese people with periodontal disease. Half of the subjects with an average body mass index (BMI) of 39 underwent gastric bypass surgery and removal of fat cells from the abdomen; this group achieved better results in treating the gums than did the control group of obese people with a BMI of 35. Treatment consisted of scaling/root planing and oral hygiene home care instructions. Further,…

Primary Stability of Self-Tapping and Nonself-Tapping Implants A study by Dr. Aleksa Markovic et al investigated the relationship between surgical techniques and self-tapping versus nonself tapping implant design in terms of implant stability in the low-density bone present in the posterior maxilla. It involved a total of 103 implants: 56 self-tapping and 56 nonself-tapping. The implants were placed in the posterior segment of the maxilla in sites prepared with either lateral bone-condensing or with bone-drilling techniques. Implant stability measurements were performed immediately after implant placement and weekly during a 12-week follow-up period. The study found that both types of implants…

Sleeping Versus Loaded Implants A study compared the outcome up to 12 years of "sleeping" versus loaded implants in the mandible, which involved 14 patients with overdentures for a total of 28 loaded implants and 14 sleeping implants. During follow-up visits, intraoral radiographs were taken to observe changes in bone level. The study found that, at each observation compared with abutment connection, the submerged non-loaded (sleeping) implants had less bone loss than their neighboring functional (loaded) implants. The difference was primarily due to more significant bone loss during the first year of loading; following the first year, changes in bone…

360° Cervical Matrix Triodent The dental profession has been notably successful in encouraging patients to maintain their teeth for a lifetime. It is common to see the majority of patients in a practice fully or almost fully edentulous, even with advancing age. The retention of the natural dentition has led to a significant rise in the lesions found in the cervical third of teeth, more from abrasion than decay (treatment is similar in any case). The Class V restoration is arguably the most efficient procedure in the dental armamentarium. Current adhesive technology allows secure bonding to both enamel and dentin…
